Facial Reshaping Surgery
Some people tend to have a well-developed jawline, which makes the face look more quadrangular or square. While a strong, square jawline can be distinctly masculine and desirable in male patients, many female patients are seeking softer more feminized shape to their face. From the view of an esthetic standard, it is more desirable for the bigonial distance (width of lower face) to be narrower than the bizygomatic distance (width of mid-face).
The size of the mandible, the volume of muscle (Masseter muscle) that surrounds the mandible, and the subcutaneous fat tissues determine the width of the lower third of the face. In a case of overdeveloped mandible, it can be corrected by bone resection, and excessive fat tissues can be removed by liposuction. However, if the main factor is Masseter hypertrophy, a treatment for the Masseter muscle itself is essentially needed.
The well-developed Masseter muscle can be congenital or acquired. The acquired etiology factors that are most responsible for Masseter hypertrophy, are believed to be clenching and bruxism. Stress can lead to bruxism (grinding your teeth at night), which can enlarge masseter. Excessive gum chewing, chewing hard items like ice or tougher meats, all can lead to an enlarged masseter muscle.
BOTOX® treatment is very effective in reducing the size of masseter muscle in patients with overdeveloped and prominent masseter. Because BOTOX® partially weakens it, the muscle becomes smaller with time. The other effect is that a weaker muscle is not able to grind like it used to, so any nighttime grinding habit is quickly broken. A total of 50-80 Units of BOTOX® is injected in both Masseters initially and patients are reviewed after one month.
A visual change of muscle volume can be seen in 2–4 weeks, with a maximum level reaching after 10 weeks. The muscle function reappears gradually in approximately 5-6 months, when it may be repeated. In the masseter muscle repeated BOTOX® injections over time can actually reduce the size of this large muscle improving external appearance and reducing muscle-clenching pain symptoms due to Bruxism.
There are various advantages of BOTOX® treatment. It is a therapy that requires simple injection with rapid effect, results generally appearing in 2-3 weeks. The injection takes about 5 minutes and there is no inconvenience to daily life after treatment. There are few side effects and little pain after treatment. The cost and discomfort are much less compared to that of masseter muscle resection or mandible bone resection.
