Buccal Fat Reduction Surgery

Removal of cheek fat pads is effective in providing more definition to the midface area. Buccal or cheek fat pad removal creates a slight hollow below the cheekbone creating a ‘Ogee or S curve’ for the cheek, considered a sign of beauty. Buccal fat pad removal is preferred for face over Liposuction, which may cause rippling, as facial fat layer is very superficial. Many people start to lose their facial fat in mid 20s but some have genetic predisposition to have heavy, fuller cheeks for a very long time.

Cheek fat pad removal is meant for this group of people who are not prone to age related facial fat loss and whose parents had heavy cheeks for fairly long period of time.

The reduction of Buccal fat pads usually is performed upon a patient either under Local anaesthesia with sedation or under general anaesthesia. It is a day care procedure and there is no need for hospital admission. The procedure of Buccal fat removal involves injection of local anaesthesia solution on the inner cheek area and a small cut is made on cheek from inside of mouth. This cut on the inner cheek is made opposite the second molar tooth. The cheek muscle or Buccinator muscle is gently separated and Buccal fat becomes visible. At this point pressure on the cheek is put from outside with surgeon’s fingers and the cheek fat starts protruding from the cut inside the mouth. This fat is gently pulled out with surgical forceps and required, equal amount of fat is removed from each side. The incision is closed with self-absorbing thread, which dissolve in 2-3 weeks time. There are no visible scars and incisions in the oral cavity heal very fast.

After surgery, patient is advised to sleep with head elevated for 2-3 days to reduce post operative swelling. The swelling and bruising generally subsides to great extent in 6-7 days. Liquid or soft diet is advised for 2-3 days to avoid chewing and associated discomfort. An antibacterial mouthwash is used for many times a day to keep mouth clean. A chinstrap or pressure garment for 2 weeks can help in reducing swelling quickly.

Buccal fat removal is a very delicate procedure to reestablish mid face contour and should be done by an experienced plastic surgeon.
